I acknowledge my many students who have discovered most of the errors that I have found to correct according to the above table. In addition to PhD students at NTNU (Norwegian University of Science and Technology), also students participating in international, two-week, WAVETRAIN courses arranged in 2005 at Chalmers University of Technology, Gothenburg, Sweden, and in 2010 at NTNU, discovered many of the errors listed in the above table.
