Comments

General comments.

I started my PhD work in 1997, with studies of the long axis function of the heart. My Supervisor was professor Terje Skjærpe. In the course of the work I was lucky to work in the interdisciplinary collaborative environment of technical ultrasound and cardiac physiology in what now is the department of circulation and imaging) in the faculty of medicine, NTNU.

Thus I collaborated with Andreas Heimdal and Hans Torp in the development of tissue Doppler based deformation imaging. The technical development was, of course an engineering project, and the method was related to the velocity gradient published by others, but the technical implications was new.

 

My specific contribution was the idea that deformation should be measured in the longitudinal direction, which was the breakthrough, making all segments of the left ventricular myocardium available for analysis through the apical window, as well as increasing the feasibility of tissue Doppler analysis. This has made deformation analysis applied as a tool world wide. I also participated in the discussion leading to the now universal colour map for deformation imaging, in use worldwide, not only in tissue Doppler, but all modalities.

 

My primary research was in validating the method, I participated in the first paper on the method, where the method was described technically, and I did a clinical feasibility pilot study in patients (Heimdal et al 1998).  I conducted further validation studies in patients, against B-mode echocardiography (Stoylen 1999) and coronary angiography (Stoylen 2000). Those were the first three publications on strain rate imaging. Validation in experimental models were done at other centres, and published from 2000 and onwards.

Further work in myocardial infarction diagnostics has been in testing the difference between motion (velocity) and deformation (strain rate) in infarction (Støylen 2003).

 

Later work has been in testing the development of automated analysis together with my first doctoral student Charlotte Bjørk Ingul, and the value of the application in stress echocardiography, with in collaboration with Dr. Th Marwick  of Brisbane University (Ingul 2007), and in looking at infarct recovery (Ingul 2005, 2010).

 

In collaboration with the department of pediatrics, Ullevål university hospital, Oslo, where I was technical co supervisor, we tested the application of deformation imaging with tissue Doppler derived strain rate to the ecamination of cardiac function in newborns, looking at optimisation of probes and settings, as well as showing defomation imaging to be more sensitive for asphyxia than conventional echocardiography  (Nestaas 2008, 2009, 2011)

 

In the technical environment at the Department of circulation and imaging (Svein Arne Aase and Hans Torp), we have developed and tested the automated detection of the aortic valve closure by tissue Doppler, and event that is important in timing the regional and global events of the heart cycle and detecting delayed contractions in diseased segments, as well as giving new insight into cardiac physiology (Aase 2006, 2008, 2010).

 

I have also tested the 3-dimensional reconstructed surface rendering of deformation data (Støylen 2003), a method that is promising in being able to measure infarct size directly by mapping the area of reduced function, instead of indirectly by reduced global function (Thorstensen 2012).

 

For the third wave if the HUNT study, I designed the HUNT echo substudy, which was performed by my PhD student Håvard Dalen, collecting normal data for strain and strain rate in healthy subjects, in order to establish the normal range, and relate to known risk factors in healthy subjects (Dalen 2010, 2010, 2011). With 1266 healthy subjects of both genders and all ages above 20, it is the largest normal strain and strain rate reference population in the world.

 

I have also been particularly concerned about dissemination of the knowledge of deformation imaging, the ability, the documentation, and especially the limitations and pitfalls of the methods of echocardiography in general and deformation imaging in special.

 

For this, i have designed a comprehensive web site that is freely accessible on the internet. The site attempts to integrate the technical/mathemathical, physiological and clinical infromation, and with heavy emphasis on problems and pitfalls of the methods. This site is used world wide, and has an average og about 10.000 hits per year. It can be acessed through

My second field of research is the effects of exercise on the heart.

 

I started collaboration on this with Ulrik Wisløff already in  1999, and have participated in studies with tissue Doppler and long axis function in normals (stoylen 2003), in myocrdial infarction (Rustad 2008), as well as doing studies in cardiac rehabilitation training (Karlsen 2008, 2009, Helgerud 2010, Aamot 2010). In this context, I designed a study, showing that high intensity interval trining had a better effect in reducing blood pressure in moderate hypertension than moderate exercise. Maybe the most important study is the demonstration of reverse remodelling in heart failure with high intensity exercise (Wisløff 2007), which now is being extended into a multicentre study (Støylen 2011).
